Either Sales Price or Est. Value (this will let us know if it's a purchase or refi)

Loan Amount

Borrower's Name

Contact Name and Phone Number

Payment method -- COD or Bill

In the comments section, put the following:

Lender's info (include name of broker, address, office number, fax number, email)

Whether the property is vacant with a lockbox

Any credit backs to the seller (NRCC)

Mention if you need a rental survey
